Online Course Forms

Grade Changes and Course Change Forms

2019-2020 Schedule Change Request Form - This form must be completed by the deadline for each trimester. Please note that all requests cannot be honored due to class size, grade level or pre-requisite courses not being completed. Click here to complete the course request change form.

TA (Teachers Assistant) Request Form - This form must be completed with your teacher. Juniors and Seniors with a minimum GPA of 3.0 are eligible for TA. Click here to download the TA (Teachers Assistant) Request Form.
