Athletics and Activities
Registration Information
The Spring Sports Orientation/Registration meeting for athletes and their parents will be held on Monday, March 1st in the FAC at 6:30pm. Please plan to attend this meeting with your student athlete. The Activities Office will be open for registrations at 6:00pm.
Registration Forms
If this is your first sport this school year, you will need to download & print the registration packet and have each form signed by both the athlete and a parent/guardian then turned in to the SLPHS Activities Office with the appropriate sports fee. We do not accept credit or debit cards at this time. Please be sure the athlete's sports physical is current (sports physicals are good for 3 years). If this is your second or third sport this school year, you will only need to fill out the emergency card and the team rules.
